Contact and scheduling
The appointment request form collects your name, contact details, preferred contact method, general availability, and consent to be contacted. These details are stored in the practice’s password-protected administrative inbox so the practice can follow up. Do not include diagnoses, treatment details, or personal history.
Website administration
Authorized practice administrators can view and manage requests. The website is hosted on Vercel and stores requests in Neon Postgres. If email notifications are enabled, Amazon SES processes scheduling notifications. Requests remain in the inbox until an administrator deletes them. Website activity and cookies
The site counts page views, booking-link clicks, and appointment requests in daily totals. Referral links may identify a referral source, such as a shared flyer. The analytics system does not store individual visitor activity records, names, raw IP addresses, or device profiles. Hosting providers process technical request information to operate the website. Temporary, one-way security identifiers help limit abuse. The site uses session cookies for administrator sign-in and does not use advertising cookies.
